Complete the following steps for establishing a new service.

Complete the Sewer Service Application (Application fee will be added to first month's bill.)
Please download the application form and return via email to info@integrawater.com or postal mail to P.O. Box 10127, Birmingham, AL 35202

Complete the Release of Water Consumption Form and Customer Agreement Form;
Print the Sewer Service Application, Release of Water Consumption Form and the Customer Agreement form. Sign and Fill in all 3. Mail the original completed documents to:
